That late in the evening your best bet may be a Taxi hire, USD45 is about right, it beats staying in Managua a night, a hotel would cost that uch anyway for something decent in a safe Barrio close to the Shuttles anyway- In regards to the second part, from Granada to Ometepe, you can bus to Rivas, or Taxi hire to San Jorge, where the ferry departs every few hours to Ometepe, last one departs at 330PM.
